We make requesting an appointment easy, with two options for requesting an appointment. Note that your insurance company may request a referral before you make an appointment with AG.

No Show/Late Cancellation

Please notify us within 24 hours of an office visit or 48 hours of a procedure if you need to cancel your appointment. Otherwise a fee of $25 or $50 will be applied to your account, and the charge must be paid prior to your next appointment.
